Testing cannot proceed if the DNA testing consent form is not signed. If the DNA testing consent form is not signed, we will send an email notification to you with a copy of the consent form for signature. You will need to sign the consent form, scan it, and email or fax it back to the laboratory so that that testing can proceed.
